I understand that the employer does not unlawfully discriminate and no question on this application is used for the purpose of
limiting or excusing any applicant from consideration on a basis prohibited by applicable local, state, or federal laws.

I understand that I will be responsible for any claims that I may have including any deductibles on insurance and insurance premiums that are obtained through the company. I understand that I will need to pay $50.00 at the beginning of each month for insurance and Worker’s Compensation. I also understand that I am responsible for the first $2,500.00 of the money owed for each claim that I am accountable for.

If I am used, I understand that I am free to resign at any time, with or without cause and without prior notice, and the company reserves the same right and does not guarantee work will always be available. This application does not constitute an agreement or contract for work for any specified period or duration. I understand that no supervisor or representative of the employer is authorized to make any assurances to the contrary and that no implied, oral, or written agreements contrary to the foregoing express language are valid unless they are in writing and signed by the employer’s President.

I also understand that I may be required to submit to any lawful drug, alcohol, or integrity test that may be required as a condition of my service or continued service. I also understand that if I am used, I will be required to provide proof of identity and legal authority to work in the United States and that federal immigration laws require me to complete an I-9 Form in this regard.
